All participants must have a current physical form on file before participating in any practices.

The 2024 CVYFCA form can be found on our website, www.cvbucks.com. Go to the "For Parents" tab and click on "Forms". You may also use a generic physical form from your provider as long as it states the participant is clear to play football and is signed by a licensed physician.
