2024 CVYFCA Football and Cheer Uniform Policy
All Football and Cheer equipment and uniforms will be due at the end of the season on  specified return dates.  We will provide parents/players 3 dates and times to return all football and cheer equipment and uniforms.  In order to receive your refunds for equipment and uniforms, you must return all items on the assigned dates.  Those that return any cheer or football equipment and uniforms after the return dates will NOT be eligible for the equipment refunds.  Parents will be liable for any missing equipment or uniforms and will be billed accordingly.

CVYFCA  SPRING FLAG FOOTBALL ACCIDENT WAIVER AND RELEASE OF LIABILITY FORM

• CVYFCA requires all participants who participate in the flag program to print and sign this waiver. Flag football starts Saturday, April 2nd at 10am. Practice sessions will be held at Lancaster Mennonite-Locust Grove Campus, located at 2257 Old Philadelphia Pike, Lancaster, PA 17602

CVYFCA Concussion Fact Sheet:

 •  CVYFCA requires all participants and their parents/guardians to read this document prior to registration. A waiver acknowledging this document has been read and understood by both participants and their parent/guardian is required to complete registration for CVYFCA programs.

CVYFCA Medical Form:

  •  All CVYFCA tackle players are required to have an annual sports physical with a signed medical release prior to participating in any practices or games. Participants who do not have a signed medical release from a licensed physician will not participate until this requirement is fulfilled.
